On-line Completion of VIP Registration Form

1.      Go to http://www.eayso.org

2.      If you have not used the program before, click on “First Time”.

3.      Read the instructions and click on next to move forward.

4.      Click on “I want to register my children to play” Parent button.

5.      You are advised to get user name and password such that the registration information is stored for future use.

6.      The local soccer Region is 102.  Enter the number or select when presented.

7.      When completing the forms, remember to scroll down to see the entire form.

8.      When you have completed the registration screens you must:

8.1.  Print out THREE copies of the form.

Click on “Print Forms”

Click on the print icon on the page, not your web browsers print icon.

When the form displays, click where indicated to print.

Repeat.

8.2.  Sign ALL copies.

9.    Go to - Player Profile Form.

10.    Print the Player Profile Form.

11.    Complete the form.

12.    Make two copies of the Player Profile Form.

13.             Bring to registration:

    Signed forms, Player Profile Forms,  &  Payment
