Print Blank VIP Registration Form

1.      Go to - Player Registration Form.

2.      Print the blank registration form.

3.     Complete the registration form.

4.      Sign the registration form.

5.      Make two copies of the signed form.

6.     Go to - Player Profile Form.

7.     Print the Player Profile Form.

8.     Complete the form.

9.    Make two copies of the Player Profile Form.

10.             Bring to registration:

    Signed forms, Player Profile Forms,  &  Payment
